During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.  We accept most insurances.

Please arrive early so that we can complete your initial paperwork. Also, please bring the following:

  • Patient's insurance card
  • List of current prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about patient's medical and surgical history
  • Recent test results, x-rays, or relevant records

Insurance and Payment Information

You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.

 

Policies

Our business is our pleasure and we value it. We ask that you respect our business scheduling policies so that your experience is magical and unforgettable! Your appointment is reserved specifically for you so that the therapist has the appropriate time to give you an amazing experience. We understand that sometimes life happens. Therefore, we respectfully require at least 24 hours’ notice for changes or cancellations of services. Late cancellations and “no-show” appointments cost us precious time and money and your visit could have been given to someone that we turned away.

Should we receive less than 24 hours’ notice of a cancellation, 50% of the treatment value will be charged.

“No-Shows” will be charged 100% of the treatment value.

We completely understand things can happen last minute and we truly are sympathetic to circumstances.

If you have been diagnosed with Covid, we ask you to submit a dated & current note from the doctor (AT HOME TESTS ARE NOT APPROVED).

We are happy to rebook your appointment for you.As a courtesy, make sure to call us when you will not be able to make your booked appointment.

We will no longer be accepting expired gift certificates. Please check the date on your certificate.

 

LATE POLICY

We realize that occasionally conditions emerge, and life and traffic are eccentric. Having a relaxing experience is of the utmost importance to you and us!

We REQUIRE that you arrive for your appointment 10 to 15 minutes before your start time to guarantee you have your full apportioned time. In the event that you are late to your appointment, we cannot guarantee any added extra time. Our next client’s appointment time could be following right after. Understand that we are committed to offering you the highest possible level of service during your appointments with us.

Please be aware your appointment incorporates dressing and consultation during the meeting time.
